Skin tears

Skin tears are acute/traumatic wounds which commonly occur in those with fragile skin, i.e. the elderly or very young.
 
Identification of at risk patients is key to reducing their incidence. Where present, skin tears should be thoroughly assessed, risk factors for further wounding eliminated where possible, and the injury classified using a recognised tool.
 
Accurate documentation should be completed to allow a consistent approach to ongoing management by all caregivers, which includes use of an appropriate wound dressing.

As they can become chronic and cause considerable distress to patients, healthcare professionals need to be able to identify those at risk, and know both how to manage injury and minimise any risk of infection.
